三、康复护理
1.功能锻炼
在医护人员指导下,根据骨折不同阶段,各关节向各方向进行主动运动、被动运动及用力抓、握等练习。
(1)伤后1~2周:
此阶段功能锻炼主要以伤肢握拳、伸指、分指、肘部屈伸为主,通过促进血液循环,达到消肿止痛的目的(图1-1-16)。伤侧肩部忌做外展、耸肩、提重物等动作,防止骨折端移位(图1-1-17)。

(2)伤后3~5周:
此阶段的功能锻炼,在不影响骨折稳定性的同时,可做伤侧肩关节前屈、后伸及托扶内收、外展,前臂内旋、外旋,活动范围以不引起疼痛为主;仰卧位时,肩关节保持外展位为主(图1-1-18)。外展型肱骨近端骨折伤肩忌外展,内收型肱骨近端骨折伤肩忌内收,动作参见肩部骨折卧位示意图(图1-1-11)。
(3)伤后6~8周:
去除外固定早期,可行主动运动、被动运动、助力运动和关节主动牵伸运动,如外旋、内旋、肩前上举、后伸、内收、外展、外展上举,以恢复肩关节活动度(图1-1-19)。
(4)受伤9周以后:
此阶段骨折稳定,可行甩肩、爬墙、摸头、摸对侧肩等锻炼,并可加大活动范围,防止肩关节僵硬(图1-1-20)。

2.辅助器具穿戴使用
(1)肩外展外旋式矫形器
1)穿:
将外展海绵软包置于腰部,前臂放在海绵软包上固定于功能位,连接腰带,调整松紧度,扣好固定;调整肩带和腋下衬垫,调整松紧度,扣好固定;将伤肢前臂和上臂固定带粘贴于软包上,固定前臂,防止前后移动(图1-1-21)。

先调松松紧带,脱健侧再脱伤侧。
(3)肩外展矫形器
适应证:肩部骨折后期需要进行康复训练者,需在医师指导下使用此矫形器。
1)穿:
协助者协助调整胸带、腰带、肩带、手臂带,贴身放好矫形器,将伤肢摆放在外展60°~70°位,分别连接好腰带、胸带、肩带、手臂带,调整松紧度并固定好(图1-1-23);此支具侧方有个钢条区域,根据康复计划,可适当进行内旋、外旋、前屈、后伸等体位进行康复训练(图1-1-24),以达肩关节最大活动范围。

2)脱:
脱去时,由协助者先解开手臂固定带、腰带,再解开胸部固定带,取下支具即可。
(4)矫形辅助器具的维护与保养:
肩带、腋下带、衬垫、腰部固定带、肘部固定带、前臂固定带、腰部衬垫、手臂衬套均为可拆卸、清洗,晒干后即可使用;海绵外展枕尽量不沾水,防潮放置;尽量减少承托重物等,免变形;魔术贴易失去黏性,尽量减少清洗次数;插件处易存留污垢,及时擦拭,保持清洁。
3.居家康复护理
(1)穿、脱衣物:
先穿伤侧肢体,再穿健侧;脱衣服时顺序相反,先脱健侧,再脱伤侧。患肢衣袖可剪开缝绑带或魔术贴,以方便穿脱(图1-1-25)。

(2)洗漱、如厕:
建议使用伤侧手进行,增加肢体的灵活性和逐渐扩大活动范围。若伤侧手不便,方向相反。
(3)梳头:
建议单手梳理或助手协助完成;长头发者,单人操作,视患侧肩关节活动度情况而行,可将头倾倒向患侧肩部,患侧手抓头发,健侧手绑扎完成。若患者肩关节活动度不足,可请协助者完成。
